Continu-Care Maternal-Child Health Programs

Maternal-ChildMultiple programs designed to support expectant mothers, their newborn babies and children, and family members.

MSS, Maternal Support Services

This program provides support and education to the pregnant Medicaid mother. Visits are made by a Registered Nurse, a Medical Social Worker and/or a Dietitian, depending on the family needs and interests. We can provide ten (10) visits during the period extending from pregnancy through 60 days after delivery.

ISS, Infant Support Services

This program provides support and education to Medicaid families with babies and children. Visits are made by a Registered Nurse, a Medical Social Worker, an infant Mental Health Specialist and/or a Dietitian. We can provide nine (9) initial visits and nine (9) additional visits from birth through age one year or more.

Grant-Funded Programs

P.A.T., Parents As Teachers Program

Parents are children's first and best teachers. We provide parents with research-based information and help them be successful first teachers. We share recent research on neuroscience (brain development) and related education to help parents foster their children's healthy brain development. This program offers:

  • Home Visits
  • Well Baby Check-ups
  • Immunization Reminders
  • Growth and Development
  • Assessment and Screenings
  • Developmental Activities
  • Handouts and Parenting Magazines
  • Recommendations on Books and Toys

This program is for all Newaygo County and Hesperia School District residents, pregnant mothers and children 0-5 years of age. Because it is grant-funded, the program is free.

Early On Program

We also serve as Early On case coordinators for special needs children.

EMD, Early Maternal and Newborn Discharge Programs

When mothers and babies are discharged soon after birth, their care can be easily and effectively accomplished in the home. We can identify and care for problems that may occur during the early post-partum period. Our nurses can assist parents 24 hours a day and answer their questions regarding:

  • Basic Infant Care
  • Feeding Problems
  • Cord Care
  • Circumcision Care
  • Jaundice or Skin Color Changes
  • Fever or Infection

For infants with more significant problems, we can provide help with:

  • Apnea Monitoring
  • Phototherapy
  • Tube Feedings

We are preferred providers for the following insurance companies:

  • Priority Health
  • Blue Care Network
  • Blue Choice

Pregnancy Related Medical Services

Continu-Care offers these and other home services:

  • Prenatal IV management of extreme pregnancy- related vomiting and kidney infection
  • Assessment and education regarding pregnancy related diabetes and pregnancy-induced high blood pressure
  • Follow up assessment and instruction on premature labor and pre-eclampsia
  • Post-Partum
  • Wound Care Dressing Changes
  • Breastfeeding Difficulties
