Homebound Care
This service is provided especially for those not bed bound but those unable to easily leave their homes. Homebound criteria means patients only leave home if:
- They have a doctor’s appointment
- They are attending Religious Services
- They need to leave for very short, infrequent period of time
Medical Assistance Management (MAM)
Continu-Care staff professionals now have the capability to assist patients in setting up and monitoring, with a new device, medications from your home. Rather than patients having to return to the hospital or long-term care facility for medication oversight, they can remain at home. This new service was underwritten by the Fremont Area Community Foundation.
