CONTINU-CARE MATERNAL/CHILD SUPPORT

Maternal/Infant Health Programs

Continu-Care Home Care Services is certified through the state of Michigan to provide the Maternal Infant Health Program (MIHP) to pregnant women from conception to eight (8) weeks after delivery, and to your infant from birth to one (1) year of age. Our goal is to help you have a healthy pregnancy and delivery experience; plus provide you with the knowledge to promote a safe and nurturing home for your new baby. To be eligible you must be covered by Medicaid. This program is FREE and intended to assist your health care provider in the management of your family’s health and well-being. Our team consists of a Registered OB/Pediatric Nurse, Nutritional Specialist, and Medical Social Worker.


Maternal Services

Provided in the comfort of your home or at a location agreeable to you and your team:

  • Prenatal teaching about your changing body, and growth and development of your baby
  • Childbirth education classes (paid for by MIHP)
  • Nutritional support and education
  • Breastfeeding support and education
  • Newborn care
  • Information on community agencies that will help find necessary baby supplies and other resources to maintain a stable home for your baby
  • Assistance with arraigning transportation to medical appointments
  • Support to stop smoking
  • Fact sheets, coupons, product samples, and magazines

Infant Services

Provided in the comfort of your home:

  • Teaching normal newborn behaviors
  • Instruction in signs and symptoms that should be checked by the doctor
  • Infant growth and development
  • Information on nutritional education and support
  • Breastfeeding education and support
  • Immunization education
  • Family planning information
  • Support to stop smoking
  • Assistance with arranging transportation to medical appointments
  • Information on community agencies that will help find necessary baby supplies and other resources to maintain a stable home for your baby
  • Fact sheets, coupons, product samples, and magazines

Grant Funded Programs

Parents as Teachers Program (P.A.T.)
This fun program helps you be your child’s best, first teacher. Our parent educators will show you what to expect as your child grows and how important you are in your child’s development. This program is FREE. Any child in Newaygo County or Hesperia School District from birth to eight (8) years is eligible for:

  • Home visits and regular assessment of your child’s developmental level, weight, and height
  • Activities to help your child develop
  • Reminders to get your child’s immunizations on time
  • Parent group meetings
  • Fact sheets and parent magazines
  • Tips about good books and toys
  • Assistance with community resources when applicable

Early On Program
For parents of children with special needs. Each child grows and learns at a different rate. The first three (3) years are key to a child’s development. Families, doctors or agencies can recommend that children up to three (3) years - who seem to have special needs or medical conditions - receive these services. Early On helps families find health care givers, teachers, and other service people to work in the family home with your child. Our experts help connect you with these services.

Medical Programs
Early Maternal Discharge (EMD)
Because mothers leave the hospital so soon after the birth of their baby, our nurses will visit your home and help identify early problems, provide education, and answer questions about:

  • Basic newborn care
  • Normal newborn behaviors
  • Newborn jaundice (yellowing of your baby’s skin and eyes)
  • Fever
  • Infections

You will receive a telephone call the day you are discharged from the hospital and one (1) or two (2) home visits. We will see you in your home so you can continue to rest and love your new baby. To be eligible for this program you must be covered by medical insurance, such as Priority Health or Blue Cross.

Skilled Nurse Care
At times during pregnancy - after the baby is born or while your child is growing - significant medical problems may occur that call for nursing care. Our nurses work with doctors and the hospital to treat those medical problems in the comfort of your home.

Maternal/Child Medical Home Care Services include the care of:

  • Extreme throwing up during pregnancy
  • Diabetes
  • High Blood Pressure
  • Preterm Labor
  • Pre-Eclampsia or Pre-Toxemia, a serious condition that may develop during pregnancy
  • Newborn Jaundice
  • Asthma and other breathing problems
  • Slow weight gain in infants
  • Feeding problems
  • Care of serious wounds
  • Antibiotic therapy through an IV
