Medical RecordsRelease Of Medical Records Information

Patients may obtain their medical records by coming into the Medical Records Department of Gerber Memorial Health Services. At that time the patient will be asked to sign a Release of Information form. A Driver's License or another form of picture ID is required for identification purposes. If possible, please try to notify us in advance by calling (231) 924-1423 so that the records can be ready for you.

The fee for obtaining records is $5.00 for the first three pages (includes records search), and $0.50 for each additional page. Patients may obtain their records Monday through Friday from 8:00 a.m. - 4:30 p.m.

Birth Certificates, Paternity Papers, Social Security Card

Birth Certificates
Paperwork for Birth Certificates is provided for newborns at birth. If a child is born on Saturday or Sunday, the parents are required to come into the Medical Records Department to sign the Birth Certificate.

**To obtain a certified copy of an infant's Birth Certificate, parents are required to contact the Newaygo County Clerk's Office at (231) 689-7235. The County Clerk's Office is open Monday through Friday 8:00 a.m. - 12:00 p.m. and 1:00 p.m. - 5:00 p.m.

Paternity Papers
In the case of a single parent, Paternity Papers will be offered for the parents to fill out at the time of the child's birth. Both the child's mother and father need to be present for this paperwork to be completed. A copy of the Paternity Papers will be given to the parents upon completion.

Social Security Card

A document indicating that a Social Security Card will be issued from the Social Security Department in Lansing, will also be provided for the parents when Birth Certificate paperwork has been completed.
